UCSF Benioff Children's Hospital Oakland - CHO (Children's Hospital of Oakland) Referrals


CHO is now called UCSF Benioff Childrens Hospital Oakland - so keep that in mind when reading these documents.

The specialists are CHO are amazingly helpful if you call them, and on occasion they even respond to emails. Here is their referral directory.

Regardless of insurance type, please complete a UCSF Benioff Oakland (CHO) referral form Please also read the CHO Referral Overview, below, which was copied and pasted from a letter from CHO dated April, 2010.

Some clinics (PFT, mental health, child development, diagnostic imaging) have clinic specific referral forms

Any child whose insurance is Contra Costa Health Plan will need a prior authorization completed, too (order this on ccLink)

For urgent referrals you need to call the specialty department directly.

You can help the parents find CHO by printing directions and maps to their various campuses.

Some CHO specialists have clinics in Brentwood and Walnut Creek. Each month, CHO publishes calendars of those clinics. As of 2014 (I will not have time to update this info often, nor do I know the wait time), they had the following clinics (some only once per month):
  • Brentwood: ENT, Cardiology (echo), Surgery, GI, Audiology
    • Walnut Creek: Nutrition, Cardiology, Orthopedics, Neurosurgery, Gastroenterology, Surgery, Nephrology, Pulmonary, Psychiatry, Endocrinology, Sports Medicine, Plastic surgery/Hand, Neurology, Occupational therapy, Behavioral pediatrics, Rheumatology. dermatology, psychiatry, endocrinology, orthopedics, psychiatry, orthopedics, neurosurgery. There is also a Sports Medicine Center in Walnut Creek.

Someone compiled specialty guidelines (I do not know how current they are), on what you may need to do help refer a patient.

To New Patient Referral Staff:
Children’s Hospital & Research Center of Oakland has implemented a fax-based referral management system to prevent lost faxes and improve the efficiency of receiving your faxed referrals. The fax number is 510-995-2956
Complete the referral form and include the following required paperwork for new patient referrals;

1. Completed Patient Evaluation and Referral Form
2. Insurance Authorization for initial visit if HMO
3. Copy front and back of Insurance Card
4. Demo/Face sheet
5. Last clinic note
6. Recent Labs results
7. Growth Chart

Your fax to Children’s is stored on a secure server as a PDF. When we receive your fax, a fax confirmation is sent thanking you for the referral. The confirmation will include the patient name, DOB, referral department, and the date and time we received your fax. Your referral is then routed to the appropriate department for processing

DO NOT send urgent referrals to the RMO fax number. Please call the specialty department and have the referring physician speak to the specialist on call.

Your patient’s family should call the specialty department directly after one business day from the time you submit your fax. Your office staff can make this new patient appointment if time permits you to do so.
